An affidavit is a written statement confirmed by oath on stamp paper. You write the facts, sign it in front of an oath commissioner or a notary public, and the official attests it with a seal and signature.
This guide covers the affidavit format for government jobs, the stamp paper you need, how to get it attested, and three samples to copy and fill in. The main sample is the affidavit of domicile, the type a job advertisement asks for most often.
Affidavit at a Glance#
| Question | Short answer |
|---|---|
| What is it? | A sworn written statement on stamp paper, used as evidence |
| Who makes it? | The deponent, the person who swears the statement is true |
| Who attests it? | An oath commissioner or a notary public |
| What does it cost? | The stamp paper value, which the advertisement states and which varies by province |
| What do you need? | Your CNIC, the facts to declare, and the stamp paper |
What Is an Affidavit?#
An affidavit is a written statement of facts confirmed by oath or affirmation. The person who makes the statement is called the deponent. The statement is used as evidence, so it must be true.
The affidavit is written or typed on non-judicial stamp paper. The deponent signs it and gives a thumb impression in front of the official who administers the oath. That official then attests it.
An affidavit is not a certificate from a government department. No department issues it for you. You buy the stamp paper, write the statement, and get it attested. That is the whole process.
When a Government Job Asks for an Affidavit#
Read the advertisement first. There is no single standard affidavit for all government jobs. The advertisement states which affidavit it wants and on what value of stamp paper. Follow what it says.
A job advertisement can ask for an affidavit at the application stage or at the interview stage. It is usually one of the types in the table below. If the advertisement does not ask for an affidavit, do not submit one.
| Type | What it declares |
|---|---|
| Affidavit of domicile | That you are domiciled in the district named in the affidavit |
| Not employed elsewhere | That you do not hold another government job |
| No criminal record | That no criminal case is pending against you |
| Name or age correction | That the name or date of birth in your documents is correct |
The affidavit of domicile is the most common type in job advertisements. It is also the most searched type in this cluster. Sample 1 below is a complete affidavit of domicile. Our domicile certificate guide explains how to get the domicile itself.
The affidavit supports your claim in the application form. It does not replace the document it refers to. An affidavit of domicile does not replace the domicile certificate, and an affidavit of no criminal record does not replace a police character certificate where the department asks for one.
Stamp Paper: How to Buy It#
Buy non-judicial stamp paper from a licensed stamp vendor. Stamp paper has a yellow background and a barcode on the upper front. Values run from Rs 50 to Rs 1,000.
Give your CNIC to the vendor. The vendor records your CNIC number in a register with your thumb impression. The vendor then stamps the paper with the date of issue, the serial number, the value, your name, your father’s or husband’s name and address, and the purpose.
Do not state one fixed value as universal. For a general affidavit the e-stamp paper value is usually Rs 100 to Rs 500. For a notarized affidavit it is usually Rs 100 to Rs 1,200. The exact value depends on the province and the nature of the declaration. The advertisement usually states the required value, so check it before you buy.
Punjab has an online e-stamp system. Availability elsewhere varies. Do not assume e-stamp works in every province. If you cannot buy e-stamp paper where you live, buy physical stamp paper from a licensed vendor.
Buy the stamp paper before you write the affidavit. The statement is written or typed on the stamp paper itself, not on plain paper and then attached to it. Keep the vendor’s receipt with your copy.

The Format: Parts of an Affidavit#
An affidavit follows a fixed format. Every part has a fixed place on the page. The table below lists the parts in order.
| Part | What to write |
|---|---|
| Title | AFFIDAVIT, at the top |
| Deponent | Your full name, father’s name, address and CNIC number |
| Solemn affirmation | That you solemnly affirm and declare the statement is true |
| Numbered paragraphs | The facts, one fact in each numbered paragraph |
| Closing declaration | That the contents are true to the best of your knowledge and belief |
| Signature and thumb impression | Your signature and thumb impression as the deponent |
| Attestation | The oath commissioner’s or notary’s seal, signature, licence number, date and place |
Write the facts in short numbered paragraphs. Put one fact in each paragraph. Do not mix two facts in one paragraph. The official who attests the affidavit reads the paragraphs in order.
Write your name, your father’s name and your CNIC number exactly as they appear on your CNIC. A name that does not match the CNIC can get the affidavit rejected. Our documents checklist explains how commissions check names against the Matric certificate and the CNIC.

Sample 1: Affidavit of Domicile#
This is the main sample. It is the affidavit a job advertisement asks for most often. Copy it, fill in the parts in [brackets], and get it attested.
AFFIDAVIT

Sample 2: General Affidavit for a Government Job#
Use this sample when the advertisement asks for a general declaration, for example that you are not employed elsewhere or that no criminal case is pending against you. Change paragraph 1 to the fact the advertisement asks you to declare.
AFFIDAVIT

Delete any paragraph that does not apply to you. Do not declare a fact that is not true. A false statement in an affidavit is perjury, and the section below explains what that means.
Sample 3: Urdu Affidavit#
Some offices accept an affidavit in Urdu. The structure is the same: title, deponent, numbered facts, closing declaration, signature and thumb impression, then attestation.

If you are a woman, change “بیان کرتا ہوں” to “بیان کرتی ہوں” and “رکھتا ہوں” to “رکھتی ہوں”. The rest of the affidavit stays the same.
How to Get It Attested#
Go in person. The deponent must sign and give a thumb impression in front of the official. The official will not attest an affidavit signed at home and brought in by someone else.
Take your original CNIC. The official checks the CNIC before administering the oath. A photocopy alone is not enough. Bring the stamp paper with the statement already written or typed on it.
Choose an oath commissioner or a notary public. An oath commissioner administers the oath and attests the affidavit. A notary public certifies a notarized affidavit under the Notaries Ordinance 1961. The notary verifies the CNIC and may check a NADRA Verisys report before applying the seal, signature and licence number.
Check which one the advertisement wants. Some advertisements ask for an affidavit attested by an oath commissioner. Others ask for a notarized affidavit. The two are not the same, so use the one the advertisement names.
Keep a copy. Keep a photocopy of the attested affidavit with your application record. Submit the original where the advertisement asks for it. Our how to apply guide explains what to submit with the form and what to keep for the interview.
What Happens If It Is False#
A false statement in an affidavit is perjury under the Pakistan Penal Code. The punishment can be a fine or imprisonment. Say this plainly to yourself before you sign: the affidavit is taken seriously because it is a sworn statement.
A false affidavit can also cost you the job. If the department finds the statement false at any stage, the application can be rejected, the appointment can be cancelled, or the case can go to court. The risk does not end when the job starts.
If a fact in your affidavit changes, do not correct it by hand after attestation. Make a fresh affidavit on fresh stamp paper with the correct fact, and get that one attested. A corrected affidavit with handwriting over the attested text can be rejected.
Common Mistakes to Avoid#
- Using the wrong stamp paper value. The advertisement states the required value. Buy that value, not the cheapest one.
- Writing on plain paper. The statement must be written or typed on the stamp paper itself.
- Signing before you reach the official. Sign and give your thumb impression in front of the oath commissioner or notary, not at home.
- Declaring a fact you have not checked. Check the domicile number, the CNIC number and the district name before you write them.
- Using another person’s affidavit as your own. The affidavit is personal to the deponent. Copy the format, not someone else’s facts.
- Submitting a photocopy where the original is asked for. Keep a copy for yourself and submit the original where the advertisement asks for it.
- Guessing the stamp paper value. The value varies by province. Check the advertisement, and if it does not state a value, ask the office that issued the advertisement.
