A medical fitness certificate is an official document from a registered medical practitioner. It certifies you are medically fit for a government job.
It records your height, weight, chest measurements, vision and general health. The examining doctor signs it with their registration number.
For most government jobs you need it after selection, at the joining stage. Some advertisements ask for it with the application. Some reject any certificate issued after the closing date. This guide covers the format, where to get it, the fee, and the rule that gets certificates rejected.

What Is a Medical Fitness Certificate?#
A medical fitness certificate is an authorised document from a registered medical practitioner. It certifies a person is medically fit for employment.
The doctor examines you and records that you have no communicable or constitutional disease and no bodily infirmity that would disqualify you from the post. The certificate carries the doctor’s signature, official stamp and registration number.
Government jobs ask for it because the department must confirm, before you join, that you are fit for the duties of the post.
The certificate is different from a sick leave certificate. A sick leave certificate says you were unfit for work because of an illness. A fitness certificate says you are fit for work. Do not submit one for the other.
When Do You Need It for a Government Job?#
For most government jobs you need the medical fitness certificate after selection, at the joining stage. After the interview and document checking, the department checks your medical fitness again before issuing the appointment letter.
Some advertisements ask for the certificate with the application instead. Police, prison and forces posts ask for it at the physical and medical test stage, before the written test or interview. Read the advertisement. It tells you when the certificate is needed and who must issue it.
One rule catches people out. Some advertisements state that a medical fitness certificate issued after the closing date fixed for receipt of online applications will not be acceptable.
The certificate should be issued by the Medical Superintendent of the District Head Quarter Hospital or Services Hospital, Lahore, or the DHQ Hospital of the respective region. If your advertisement carries this rule, get the certificate before the closing date, from the hospital it names. A certificate from another doctor, or one dated after the closing date, is rejected.
What the Certificate Contains#

The standard physical fitness certificate records:
- Your name, age by your own statement and by appearance, and the post you are a candidate for.
- Your height in feet and inches.
- Your weight in kilograms.
- Your chest measurements on full inspiration and on full expiration.
- The acuteness of your vision.
- Your general appearance.
- Whether you are fit for outdoor work.
- Your personal marks of identification.
- Whether you have marks of smallpox or vaccination.
- Whether you have any disease, communicable or otherwise, or any constitutional affection or bodily infirmity, and if so, what it is.
The examining medical authority then signs the certificate with their registration number. It states whether the condition found is a disqualification for the employment or service you seek.
Some departments give you their own medical form. The form asks for your personal details, blood group and a passport-size photograph attested by your consulting doctor.
It also asks for a candidate statement: any congenital defect or abnormality, any physical deformity or handicap, any serious illness or operation in the last two years, and any treatment for cancer, tumour, cyst or another growth. Fill it in completely and hand it over to your induction coordinator. Keep the attested certificate with your application file. Our documents checklist lists every document a government job application needs.
Where to Get a Medical Fitness Certificate#

Go to a District Headquarters (DHQ) Hospital and ask for the Medical Superintendent. The Medical Superintendent issues the certificate after an examination. Services Hospital in Lahore also issues it.
A general fitness certificate can be issued by any registered medical practitioner, an MBBS doctor with a letterhead and a stamp, at a government hospital or a private clinic. But for a government job, follow the advertisement. If it names the Medical Superintendent of a DHQ Hospital, get it from there. A certificate from another doctor may not be accepted.
Online platforms now offer medical fitness certificates from PMDC-registered doctors, often within 30 minutes, delivered by email and WhatsApp. Be careful with these for a government job. Where the advertisement names a DHQ Medical Superintendent, an online certificate may not be accepted. Use the online route only where the advertisement does not name a specific issuing authority.
For a driving licence, the medical certificate is a different form, Form B. It is attested by a medical practitioner serving in any district hospital after a medical test. Do not use a driving licence medical certificate for a government job application.
How to Get a Medical Fitness Certificate#
Take your original CNIC, two passport-size photographs, and the advertisement or appointment letter showing the post. If the department gave you its own medical form, take that too, filled in and signed.
- Read the advertisement. Check whether the certificate is needed with the application or after selection, who must issue it, and whether it must be dated before the closing date.
- Go to the named hospital. For most government jobs this is a District Headquarters Hospital. Ask for the Medical Superintendent’s office.
- Undergo the examination. The doctor measures your height, weight and chest, checks your vision, and records your general health and identification marks.
- Get the certificate signed and stamped. Check that the doctor’s signature, official stamp and registration number are all on the certificate, and that your name, CNIC number and the post are spelled correctly.
- Keep the original safe. Submit a copy where the department asks for one, and keep the original for document checking and joining.
Medical Fitness Certificate Fee#
There is no fixed official fee for a medical fitness certificate. A government hospital charges a nominal fee, and a private clinic sets its own charge. Confirm the fee at the hospital or clinic before you go.
Do not pay an agent who promises a certificate without an examination. A certificate issued without an examination is not a valid medical document, and a department that checks it will reject it.
What If You Are Found Unfit?#
If the examining doctor finds a condition, the certificate records it and states whether it is a disqualification for the post. A minor condition that does not affect the duties of the post is usually not a disqualification.
If you are found physically or mentally unfit, the case is escalated to a medical board. For federal posts, a review committee of the Federal Public Service Commission makes the final determination. Get a second opinion from a specialist if you disagree with the finding, and keep the reports. The board, not the first doctor, decides.
Annual Health Checks for Serving Officers#
Serving officers have a separate rule. In May 2025 the Prime Minister approved annual medical examinations for all federal government officers serving in grade 17 and above.
Each officer undergoes health screening at a government teaching hospital or district headquarters hospital. The report is submitted on a designated proforma by 31 March each year.
The report is recorded in a dedicated medical roll, and a certificate of completion is forwarded to the Establishment Division. The checks are meant to detect health conditions early and to judge an officer’s suitability for training, promotions and future responsibilities.
If an officer is found physically or mentally unfit, the case is escalated to a medical board, and a review committee of the Federal Public Service Commission makes the final determination.
Common Mistakes That Get a Certificate Rejected#
- Getting it after the closing date. Some advertisements reject any certificate issued after the closing date. Get it before the deadline.
- Getting it from the wrong doctor. If the advertisement names the Medical Superintendent of a DHQ Hospital, a certificate from another doctor may not be accepted.
- Using an online certificate where the advertisement names a hospital. An online certificate from a PMDC-registered doctor is valid in general, but not where the advertisement specifies a DHQ Medical Superintendent.
- Missing signature, stamp or registration number. Check all three before you leave the hospital. A certificate without them is not accepted.
- Wrong name or CNIC number. Check the spelling against your CNIC before you leave. A mismatch sends the file back.
- Using a driving licence Form B. Form B is for a driving licence, not a government job. Use the certificate the advertisement asks for.
- Paying an agent for a certificate without an examination. A certificate issued without an examination is not valid, and a department that checks it will reject it.
- Not keeping the original. Submit a copy where asked, and keep the original for document checking and joining.
- Ignoring a condition the doctor found. If the certificate records a condition, get it treated and keep the reports. The medical board decides, not the first doctor.
- Submitting a sick leave certificate. A sick leave certificate says you were unfit. A fitness certificate says you are fit. Do not submit one for the other.
